The crew <strong>mistaking a new taxiway for a runway<\/strong>!<\/p>\n<p>The crew, with thousands of hours experience, took off on a surface that didn\u2019t have runway markings or runway lights. Thankfully no one was seriously hurt.\u00a0 It\u2019s too early to exactly say why this happened, but it\u2019s clear that some sort of <strong>\u201cexpectation bias\u201d<\/strong> was a factor. Expecting to make the first left turn onto the runway. One has to ask \u2013 was ATC monitoring the take off?<\/p>\n<p>After the tragic <a href=\"https:\/\/en.wikipedia.org\/wiki\/Singapore_Airlines_Flight_006\" target=\"_blank\" rel=\"noopener noreferrer\">Singapore 747 accident<\/a> in Taipei, technology was developed to audibly notify crew if they were about to depart <strong>&#8220;ON TAXIWAY&#8221;<\/strong>. This is known as the\u00a0<a href=\"https:\/\/en.wikipedia.org\/wiki\/Runway_Awareness_and_Advisory_System\" target=\"_blank\" rel=\"noopener noreferrer\">Runway Awareness and Advisory System<\/a> (RAAS).<\/p>\n<p>Sadly the Riyadh incident is not isolated. There have been a plethora of near misses in the past few years (more details in Extra Reading below).<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignleft wp-image-5335 size-medium\" src=\"https:\/\/ops.group\/blog\/wp-content\/uploads\/2018\/08\/Screen-Shot-2018-08-06-at-23.36.37-300x222.png\" alt=\"\" width=\"300\" height=\"222\" srcset=\"https:\/\/ops.group\/blog\/wp-content\/uploads\/2018\/08\/Screen-Shot-2018-08-06-at-23.36.37-300x222.png 300w, https:\/\/ops.group\/blog\/wp-content\/uploads\/2018\/08\/Screen-Shot-2018-08-06-at-23.36.37-768x569.png 768w, https:\/\/ops.group\/blog\/wp-content\/uploads\/2018\/08\/Screen-Shot-2018-08-06-at-23.36.37-676x501.png 676w, https:\/\/ops.group\/blog\/wp-content\/uploads\/2018\/08\/Screen-Shot-2018-08-06-at-23.36.37.png 910w\" sizes=\"auto, (max-width: 300px) 100vw, 300px\" \/>There have also been more than a few \u201cincidents\u201d of aircraft from <a href=\"https:\/\/www.flyingmag.com\/news\/cause-c-17-landing-too-small-airport-revealed\" target=\"_blank\" rel=\"noopener noreferrer\">C17\u2019s<\/a> to <a href=\"https:\/\/www.ainonline.com\/aviation-news\/2014-01-06\/atlas-identifies-causes-747s-landing-wrong-airport\" target=\"_blank\" rel=\"noopener noreferrer\">747s<\/a> <strong>landing<\/strong> at the <strong>wrong<\/strong> airports! The most notable near miss recently was that of an Air Canada A320 <a href=\"https:\/\/en.wikipedia.org\/wiki\/Air_Canada_Flight_759\" target=\"_blank\" rel=\"noopener noreferrer\">nearly landing on a taxiway<\/a> full of aircraft at <strong>KSFO\/San Francisco<\/strong>. But it\u2019s happened to <a href=\"https:\/\/www.flightglobal.com\/news\/articles\/ntsb-complex-error-chain-preceded-delta-767-taxiway-336663\/\" target=\"_blank\" rel=\"noopener noreferrer\">Delta<\/a> and <a href=\"https:\/\/www.seattletimes.com\/business\/boeing-aerospace\/faa-investigates-alaska-landing-on-taxiway-at-sea-tac\/\" target=\"_blank\" rel=\"noopener noreferrer\">Alaskan Air<\/a> recently too.<\/p>\n<p>It is an even bigger issue at a General Aviation level (and not just because <a href=\"http:\/\/www.latimes.com\/local\/lanow\/la-me-ln-ford-taxiway-agreement-20170331-story.html\" target=\"_blank\" rel=\"noopener noreferrer\">Harrison Ford did it<\/a>!). The FAA safety team <a href=\"https:\/\/www.faasafety.gov\/SPANS\/noticeView.aspx?nid=7400\" target=\"_blank\" rel=\"noopener noreferrer\">recently noted<\/a>;<\/p>\n<p><em>The FAA Air Traffic Organization (ATO) has advised of an increase in, \u201c<strong>Wrong Surface Landing Incidents<\/strong>\u201d in the National Airspace System (NAS).<\/em><\/p>\n<p><span style=\"text-decoration: underline;\"><strong>Incidents include:<\/strong><\/span><\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignright size-medium wp-image-5337\" src=\"https:\/\/ops.group\/blog\/wp-content\/uploads\/2018\/08\/Screen-Shot-2018-08-06-at-23.37.46-300x172.png\" alt=\"\" width=\"300\" height=\"172\" srcset=\"https:\/\/ops.group\/blog\/wp-content\/uploads\/2018\/08\/Screen-Shot-2018-08-06-at-23.37.46-300x172.png 300w, https:\/\/ops.group\/blog\/wp-content\/uploads\/2018\/08\/Screen-Shot-2018-08-06-at-23.37.46-768x441.png 768w, https:\/\/ops.group\/blog\/wp-content\/uploads\/2018\/08\/Screen-Shot-2018-08-06-at-23.37.46-1024x587.png 1024w, https:\/\/ops.group\/blog\/wp-content\/uploads\/2018\/08\/Screen-Shot-2018-08-06-at-23.37.46-676x388.png 676w, https:\/\/ops.group\/blog\/wp-content\/uploads\/2018\/08\/Screen-Shot-2018-08-06-at-23.37.46.png 1628w\" sizes=\"auto, (max-width: 300px) 100vw, 300px\" \/><\/p>\n<ul>\n<li>Landing on a runway other than the one specified in the ATC clearance (frequently after the pilot provides a correct read back)<\/li>\n<li>Landing on a Taxiway<\/li>\n<li>Lining up with the wrong runway or with a taxiway during approach<\/li>\n<li>Landing at the wrong airport<\/li>\n<\/ul>\n<p>The FAA published some <strong>shocking statistics:<\/strong><\/p>\n<ul>\n<li><strong>557<\/strong> \u201c<strong>wrong<\/strong> surface <strong>landing<\/strong>\/<strong>approach<\/strong> <strong>events<\/strong>\u201d between 2016-2018. <strong>That\u2019s one every other day!<\/strong><\/li>\n<li><strong>89%<\/strong> <strong>occurred during daylight hours<\/strong><\/li>\n<li><strong>91% occurred with a visibility of 3<\/strong> <strong>statue miles or greater<\/strong><\/li>\n<\/ul>\n<p><a href=\"https:\/\/www.faa.gov\/airports\/runway_safety\/publications\/media\/Runway_Safety_Best_Practices_Brochure.pdf\" target=\"_blank\" rel=\"noopener noreferrer\"><img loading=\"lazy\" decoding=\"async\" class=\"alignright wp-image-5338\" src=\"https:\/\/ops.group\/blog\/wp-content\/uploads\/2018\/08\/Screen-Shot-2018-08-06-at-23.39.37-502x1024.png\" alt=\"\" width=\"200\" height=\"408\" srcset=\"https:\/\/ops.group\/blog\/wp-content\/uploads\/2018\/08\/Screen-Shot-2018-08-06-at-23.39.37-502x1024.png 502w, https:\/\/ops.group\/blog\/wp-content\/uploads\/2018\/08\/Screen-Shot-2018-08-06-at-23.39.37-147x300.png 147w, https:\/\/ops.group\/blog\/wp-content\/uploads\/2018\/08\/Screen-Shot-2018-08-06-at-23.39.37-676x1378.png 676w, https:\/\/ops.group\/blog\/wp-content\/uploads\/2018\/08\/Screen-Shot-2018-08-06-at-23.39.37.png 726w\" sizes=\"auto, (max-width: 200px) 100vw, 200px\" \/><\/a><\/p>\n<hr \/>\n<p><strong>So what to do?<\/strong><\/p>\n<p>There are numerous <em>\u2018best operating practices\u2019<\/em> pilots can use to help avoid such incidents.<\/p>\n<ul>\n<li>Be <strong>prepared<\/strong>! Preflight planning should include familiarization with destination and alternate airports to include airport location, runway layout, NOTAMs, weather conditions (to include anticipated landing runway)<\/li>\n<li><strong>Reduce<\/strong> cockpit <strong>distractions<\/strong> during approach and landing phase of flight.<\/li>\n<li><strong>Use visual cues<\/strong> such as verifying right versus left runways; runway magnetic orientation; known landmarks versus the location of the airport or runway<\/li>\n<li>Be on the lookout for \u201c<strong>Expectation Bias<\/strong>\u201d If approaching a familiar airport, ATC might clear you for a different approach or landing runway.\u00a0 Be careful not to fall back on your past experiences.\u00a0 Verify!<\/li>\n<li><strong>Always include<\/strong> the assigned <strong>landing runway and<\/strong> your <strong>call sign<\/strong> in the <strong>read back<\/strong> to a landing clearance<\/li>\n<li><strong>Utilize<\/strong> <strong>navigation equipment<\/strong> such as Localizer\/GPS (if available) <strong>to verify proper runway alignment<\/strong><\/li>\n<\/ul>\n<p style=\"text-align: center;\">It\u2019s worth spending a few minutes watching this.<\/p>\n<p><iframe loading=\"lazy\" title=\"Wrong Surface Landing\" width=\"1080\" height=\"608\" src=\"https:\/\/www.youtube.com\/embed\/5II-s_j35cI?feature=oembed\" frameborder=\"0\" allow=\"accelerometer; autoplay; encrypted-media; gyroscope; picture-in-picture\" allowfullscreen><\/iframe><\/p>\n<p><strong>Extra Reading<\/strong><\/p>\n<ul>\n<li><a href=\"https:\/\/www.scmp.com\/article\/732246\/attempted-finnair-take-hk-taxiway-triggers-investigation\">Finnair A340 taxiway rejected take off Hong Kong<\/a><\/li>\n<li><a href=\"https:\/\/www.aeroinside.com\/item\/10558\/portugalia-e190-at-nice-on-nov-6th-2017-takeoff-from-taxiway-rejected-on-atc-instruction\">Portugal ERJ-190 taxiway rejected take off Nice<\/a><\/li>\n<li><a href=\"http:\/\/avherald.com\/h?article=4272f72c\">KLM B733 taxiway take off Amsterdam<\/a><\/li>\n<li><a href=\"https:\/\/www.atrics.com\/2017\/05\/sharjah-incident-2015\/\">Schaheen Air B734 taxiway take off Sharjah<\/a><\/li>\n<li><a href=\"http:\/\/www.aviationpros.com\/news\/10404995\/faa-investigating-cargo-jet-that-took-off-on-taxiway-at-anchorage-intl-airport\">Eva Air MD11 taxiway take off Anchorage<\/a><\/li>\n<li><a href=\"http:\/\/avherald.com\/h?article=44a21dc3\">Etihad A330 lined up on runway edge Abu Dhabi \u2013 rejected take off<\/a><\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n","protected":false},"excerpt":{"rendered":"<p>It\u2019s happened again.
