{"id":11144,"date":"2020-12-14T06:12:17","date_gmt":"2020-12-14T11:12:17","guid":{"rendered":"https:\/\/ops.group\/blog\/?p=11144"},"modified":"2020-12-14T06:12:17","modified_gmt":"2020-12-14T11:12:17","slug":"tcas-trouble-why-were-getting-it-wrong","status":"publish","type":"post","link":"https:\/\/ops.group\/blog\/tcas-trouble-why-were-getting-it-wrong\/","title":{"rendered":"TCAS Trouble: Why We&#8217;re Getting It Wrong"},"content":{"rendered":"<p>Earlier this year Eurocontrol <a href=\"https:\/\/www.skybrary.aero\/bookshelf\/books\/5842.pdf\">published a report<\/a> on TCAS Resolution Advisories, and the results weren\u2019t pretty\u2026<\/p>\n<p>Over a 12-month period, over the heart of Europe, only 38% of RAs were flown correctly and <strong>34% of aircraft even manoeuvred in the wrong direction.<\/strong><\/p>\n<p>In other words, <strong>nearly half of crew for one reason or another didn\u2019t follow the RA<\/strong> \u2013 a last-resort safety net proven to save lives. So concerned are Eurocontrol, they rank the issue as its <strong>second highest air traffic threat<\/strong> &#8211; it\u2019s a big deal.<\/p>\n<h4>Here\u2019s the issue in a nutshell<\/h4>\n<p>ICAO say that no matter what, unless the safety of your aircraft is compromised by something more dangerous (think terrain or stall etc.) if you get an RA, <strong>you have to follow it<\/strong>.<\/p>\n<p>TCAS, ACAS or whatever you want to call it has been around for a long time. Development started back in the 50s, and it has been mandated in the US for larger aircraft since the 80s. It has become incredibly reliable.<\/p>\n<p>So, if it\u2019s that black and white, the question remains, <strong><u>why<\/u> does this keep on happening?\u00a0<\/strong>Turns out there are a bunch of reasons, and so it is worth taking a look at exactly what is going wrong up there.<\/p>\n<h4>The Elephant in the Room<\/h4>\n<p>We may as well address it first \u2013 when crew choose to second guess an RA. The good news is that this isn\u2019t happening very often. Most of the time there are other factors at play. But while we\u2019re here, a little note on TAs and RAs.<\/p>\n<p>Traffic Advisories (TA) <strong>prevent<\/strong>. You haven\u2019t lost separation yet, but you might. They\u2019re a warning for us to go heads up and do something about it \u2013 make visual contact, talk to ATC, level off, you name it. This is the time for us to go to work and make decisions.<\/p>\n<p>Resolution Advisories (RA) <strong>mitigate<\/strong>. There is no more time to prevent \u2013 <strong>that ship has sailed.<\/strong> RA\u2019s typically trigger when you are within 25 seconds of a collision threat with the other aircraft. But here\u2019s the kicker \u2013 you are expected to respond to it within 5 seconds.\u00a0In other words, there is not much time for us to make effective decisions. Safest course of action? Because we think <strong>we can see the threat<\/strong> <strong>out the window.<\/strong> Unfortunately, you can\u2019t assume that the aircraft you can see is the one who triggered the RA. We\u2019re also not very good at assessing threats visually, especially at altitude and it does not give us any info about what the other aircraft is intending to do.<\/p>\n<div id=\"attachment_11148\" style=\"width: 1034px\" class=\"wp-caption alignnone\"><img loading=\"lazy\" decoding=\"async\" aria-describedby=\"caption-attachment-11148\" class=\"wp-image-11148 size-large\" src=\"https:\/\/ops.group\/blog\/wp-content\/uploads\/2020\/12\/view-from-window-1024x684.jpg\" alt=\"\" width=\"1024\" height=\"684\" srcset=\"https:\/\/ops.group\/blog\/wp-content\/uploads\/2020\/12\/view-from-window-1024x684.jpg 1024w, https:\/\/ops.group\/blog\/wp-content\/uploads\/2020\/12\/view-from-window-300x201.jpg 300w, https:\/\/ops.group\/blog\/wp-content\/uploads\/2020\/12\/view-from-window-768x513.jpg 768w, https:\/\/ops.group\/blog\/wp-content\/uploads\/2020\/12\/view-from-window-900x600.jpg 900w, https:\/\/ops.group\/blog\/wp-content\/uploads\/2020\/12\/view-from-window.jpg 1496w\" sizes=\"auto, (max-width: 1024px) 100vw, 1024px\" \/><p id=\"caption-attachment-11148\" class=\"wp-caption-text\">Is this aircraft above, below or at our level? And what is it planning to do next? RAs do a lot better job of assessing a threat than our eyes can.<\/p><\/div>\n<p><strong>Startle Factor<\/strong> \u2013 Put us in a stressful situation and we react in different ways. RA\u2019s are a rare event, and they\u2019re <strong>not always preceded with a TA.<\/strong> In other words, without warning they can emerge with significant \u2018pucker factor&#8217;. A large number of mis-flown RAs in the EUROCONTOL report lasted for less than 8 seconds. Beware of the <strong>\u2018knee jerk\u2019 reaction<\/strong> &#8211; our instinct is to act but surprise can get in the way of procedure.<\/p>\n<p><strong>Beware of\u00a0<\/strong><b>Contradictions<\/b>\u00a0\u2013 It\u2019s not ATC&#8217;s fault, but it\u2019s important to understand. They don\u2019t know what your TCAS is telling you to do and they will be working hard to help. The issue is when <strong>ATC instructions contradict your RA<\/strong>. In 2002, a Tupolev passenger jet <a href=\"https:\/\/en.wikipedia.org\/wiki\/2002_\u00dcberlingen_mid-air_collision\">collided<\/a> with a 757 over Germany \u2013 one crew followed the RA and the other ATC. The industry learnt an important lesson: <strong>always follow the RA<\/strong>. Use the phrase \u201cTCAS RA\u201d on the radio and ATC will understand you are following one.<\/p>\n<div id=\"attachment_11156\" style=\"width: 740px\" class=\"wp-caption alignnone\"><img loading=\"lazy\" decoding=\"async\" aria-describedby=\"caption-attachment-11156\" class=\"wp-image-11156 size-full\" src=\"https:\/\/ops.group\/blog\/wp-content\/uploads\/2020\/12\/screen.jpg\" alt=\"\" width=\"730\" height=\"458\" srcset=\"https:\/\/ops.group\/blog\/wp-content\/uploads\/2020\/12\/screen.jpg 730w, https:\/\/ops.group\/blog\/wp-content\/uploads\/2020\/12\/screen-300x188.jpg 300w\" sizes=\"auto, (max-width: 730px) 100vw, 730px\" \/><p id=\"caption-attachment-11156\" class=\"wp-caption-text\">When a TCAS RA and ATC conflict, the TCAS takes precedent.<\/p><\/div>\n<p><strong>Performance<\/strong> \u2013 RA\u2019s are often not followed as the crew are <strong>worried about performance.<\/strong> This usually happens when they\u2019re heavy and high or near their service ceiling and get a climbing RA. So, what should you actually do? The official word is this: <strong>do your best to follow it<\/strong>, even if your response is weak. Even if it means maintaining your level. In most cases an RA will only result in a level change of less than 500 feet. A weak reaction is safer than an opposite one.<\/p><\/div>\n<p><strong>Training \u2013<\/strong> That old chestnut. But the reality is it is really important to practice these things in the sim. Weird ones. Unexpected ones. Ugly ones. Ones with multiple threats. Because this is usually what we\u2019re up against in <strong>the real world.<\/strong> Also keep your finger on the pulse for changes. Some modern aircraft can now fly RA\u2019s automatically, but the sims you train in may not have had the same update.<\/p>\n<div id=\"attachment_11155\" style=\"width: 1034px\" class=\"wp-caption alignnone\"><img loading=\"lazy\" decoding=\"async\" aria-describedby=\"caption-attachment-11155\" class=\"wp-image-11155 size-full\" src=\"https:\/\/ops.group\/blog\/wp-content\/uploads\/2020\/12\/777-sim-2.jpg\" alt=\"\" width=\"1024\" height=\"684\" srcset=\"https:\/\/ops.group\/blog\/wp-content\/uploads\/2020\/12\/777-sim-2.jpg 1024w, https:\/\/ops.group\/blog\/wp-content\/uploads\/2020\/12\/777-sim-2-300x200.jpg 300w, https:\/\/ops.group\/blog\/wp-content\/uploads\/2020\/12\/777-sim-2-768x513.jpg 768w, https:\/\/ops.group\/blog\/wp-content\/uploads\/2020\/12\/777-sim-2-900x600.jpg 900w\" sizes=\"auto, (max-width: 1024px) 100vw, 1024px\" \/><p id=\"caption-attachment-11155\" class=\"wp-caption-text\">Practice all the nasty ones &#8211; reversal RAs, multiple aircraft, low and slow, high and heavy &#8211; you name it.<\/p><\/div>\n<p><strong>Older<\/strong> <strong>Versions<\/strong> &#8211; watch out for them. The latest one (7.1) has a number of major safety updates including clearer instructions and &#8216;reversals&#8217; &#8211; a fancy term for knowing when the other aircraft isn&#8217;t doing what it is supposed to do. Older versions of TCAS are more likely to be misunderstood by crew. One phrase in particular is especially bad &#8211; &#8220;Adjust Vertical Speed, Adjust.&#8221; In many cases crew have increased their vertical speed rather than reduce it. But it is only as reliable as the humans who respond to it.
